2010 Forms & Applicant Information

The 2010 applicant cycle closed on August 3, 2009. Check back in December for information and deadlines for the 2011 applicant cycle.

Forms needed for students utilizing the commitee process:

• Checklist PDF or available as a word DOC
• Resume Template - *REMEMBER* to upload to Blackboard as ONE document with Personal Statement & Self-Reflection Questions. (The resume template is a guideline, you may use your own resume if you choose)
• Personal Statement: found in the Student Inventory
• Self-Reflection Questions: found in the Student Inventory
• Letter of Recommendation Waiver Form: PDF or WordDoc
• Student Conduct Certification
• Official Transcripts from all Colleges and Universities attended.

Credentials Mailing Form please submit this AFTER receiving secondaries

Forms needed for students only using the credentials service:

• The Credentials Only form
• $20 credentials fee: check or money order, Payable to University of Maryland. NO CASH will be accepted
• Credentials Mailing Form
• Letter of Recommendation Waiver Form.  Please note that if the waiver form is not submitted, the default option is that you have retained your right to view your letters and thus we will stamp your letters “non-confidential”.
